Access the application by clicking on the application link below.  The application is in portable document format (pdf) format.  You will need Adobe Acrobat Reader to view this file.  After downloading Adobe Acrobat Reader, read and follow the installation instructions on Adobe's download page. Once Adobe Acrobat Reader is installed, you will be able to view and print .pdf files.

   b. Read the instructions for applying for admission on page 2 of the application.
   

c.The application can be completed by (1) printing the application and typing or writing the required information in by hand or (2) typing the information in before printing (instructions for using fill-in form).

d. Sign both signature lines and date.

Freshman Applicants:  Have your high school send ASUMH an official transcript that includes date of graduation. 

Transfer Applicants:  Request each college or university attended send ASUMH an official transcript. 

Transcript Request Form  - Access the Transcript Request form by clicking on one of the application links below.  The application is available in two formats.

   Form to request transcriptions from other colleges and universities be sent to ASUMH (.doc)

The form can be completed by printing the Transcript Request Form and typing or writing the required information in by hand.
