Requested Documents


Provide a copy of the requested documents to the Office of Scholarships and Financial Aid at Arkansas State University - Mountain Home.

Copy of DD214 Form - The student indicated a status as a veteran on the FAFSA did not match the Veterans Administration records. The student’s member copy 4 of the DD214 Form is being requested in an effort to resolve the mismatch.


Copy of Drivers License with Date of Birth Legible - The student’s name, date of birth and social security number reported may have resulted in mismatching information. The student’s driver’s license is being requested in an effort to resolve the mismatch.


Copy of INS Alien Registration Documentation - Provide a copy of Alien Registration Documentation from the Immigration and Naturalization Services (INS).


Copy of Marriage License - The marital status reported on the student’s FAFSA has resulted in mismatching information. The marriage license is being requested in an effort to resolve the mismatch.


Copy of Official Military Orders - Provide a copy of the student's Official Military Orders to confirm you answer on the FAFSA documenting that you are currently serving on active duty in the U.S. Armed Forces for purposes other than training.


Copy of Parent 1 Drivers License with Date of Birth Legible - The name, date of birth and social security number reported for FAFSA Parent 1 on the student’s FAFSA has resulted in mismatching information. The FAFSA Parent 1 driver’s license is being requested in an effort to resolve the mismatch.


Copy of Parent 1 Social Security Card - The name, date of birth and social security number reported for FAFSA Parent 1 on the student’s FAFSA has resulted in mismatching information. The social security card for FAFSA Parent 1 is being requested in an effort to resolve the mismatch.


Copy of Parent 2 Drivers License with Date of Birth Legible - The name, date of birth and social security number reported for FAFSA Parent 2 on the student’s FAFSA has resulted in mismatching information. The FAFSA Parent 2 driver’s license is being requested in an effort to resolve the mismatch.


Copy of Parent 2 Social Security Card - The name, date of birth and social security number reported for FAFSA Parent 2 on the student’s FAFSA has resulted in mismatching information. The social security card for FAFSA Parent 2 is being requested in an effort to resolve the mismatch.


Copy of Parent's 2021 or 2022 W2 Statement(s) - The parent(s) of a dependent student chosen for verification that will not file or is not required to file a 2021 or 2022 Federal Income Tax Return with the IRS MUST provide a copy of all IRS W-2’s that the parent(s) received for work performed during 2021 or 2022.


Copy of Parent's Divorce Decree - The parental marital status and income information reported on the student’s FAFSA has resulted in mismatching information. Your parent's divorce decree is being requested in an effort to resolve the mismatch.


Copy of Student's 2021 or 2022 W2 Statement(s) & Spouse's if Married - The student chosen for verification that will not file or is not required to file a 2021 or 2022 Federal Income Tax Return with the IRS MUST provide a copy of all IRS W-2’s that the student received for work performed during 2021 or 2022.

  • The spouse of a married student must also provide a copy of all IRS W-2’s that the spouse received for work performed during 2021 or 2022.


Copy of Student's Birth Certificate - The student’s name, date of birth and social security number reported has resulted in mismatching information. The student’s birth certificate is being requested to resolve the mismatch.


Copy of Student's Divorce Decree - The student's marital status and income information reported on the student’s FAFSA has resulted in mismatching information. The student's divorce decree is being requested in an effort to resolve the mismatch.


Copy of Student's Social Security Card - The student’s name, date of birth and social security number reported has resulted in mismatching information. The student’s social security card is being requested to resolve the mismatch.


Copy of Valid Government ID - A Valid photo ID CANNOT be expired and MUST be issued by a recognized United States federal or state governmental entity.

  • A Valid Government-issued photo identification (ID) is a driver's license, other state-issued ID or United States passport.


Parent's 2021 or 2022 Tax Information - A student chosen for verification whose parent(s) filed or will file a 2021 or 2022 Federal Income Tax Return must provide IRS Tax Return Transcript(s) from the IRS as part of the requested Verification documentation.

  • You can get the IRS Return Transcript (s) from http://www.irs.gov/Individuals/Get-Transcript .
  • If the IRS Tax Return Transcript(s) is not possible contact the IRS directly at 1(800)908-9946 for assistance, or if you believe you may have issues related to identity theft, call 1(800)908-4490 for help requesting an IRS Tax Return Transcript.


Proof of US Citizenship - Documentation of Proof of U.S. Citizenship is needed.

Student and/or Spouse's 2021 or 2022 IRS Tax Return Transcript - A married student chosen for verification whose spouse has filed or will file a 2021 or 2022 Federal Income Tax Return separate from the student MUST provide a 2021 or 2022 IRS Tax Return Transcript for both the student and spouse as part of the verification process.

  • To get IRS Tax Return Transcripts, go to www.irs.gov and follow the on screen instructions to set up an account with the IRS then log in to download IRS Tax Return Transcript(s).
  • If downloading the IRS Tax Return Transcript is not possible contact the IRS directly at 1(800)908-9946 for assistance, or if you believe you may have issues related to identity theft, call 1(800)908-4490 for help requesting an IRS Tax Return Transcript.


Student has One or More Loans in an Active Bankruptcy Status - A student may remain eligible for Federal Financial Aid while in active bankruptcy proceedings as long as loans that have been deferred due to bankruptcy proceedings have been in good standing (Not in Default) prior to any deferment based on bankruptcy.


Student Signature on Federal Signature Page - The FAFSA was not signed by the student. A student's signature is required on the Free Application for Federal Student Aid (FAFSA).


Transcript Needed - An official academic transcript from a post-secondary educational institution previously attended must be received and interpreted by the Registrar/Admissions Office before Federal Financial Aid may be offered.

  • Contact the Registrar/Admissions Office or the Office of Scholarships and Financial Aid to discuss which post-secondary educational institution(s) transcript are not received.

Aid Suspended - Student Appeal Required

The student is considered ineligible for Federal Financial Aid because student is not maintaining the required Satisfactory Academic Progress.

  • Student must submit a letter of appeal to the Office of Scholarships and Financial Aid at Arkansas State University – Mountain Home.
  • Documentation that substantiates statements in a letter of appeal are recommended, but not always required. The appeals committee may request documentation that substantiates statements made prior to considering an appeal.


Document Resolution of Previous Federal Aid Overpayment

Our records indicate that you have an unresolved over-payment of Federal Aid from a previously attended educational institution.

  • A Letter is needed from the previously attended educational institution reflecting you no longer owe an over-payment.
  • Students can view their Federal Student Aid status by logging onto https://studentaid.gov/ and reviewing your Dashboard.


Documentation that all Federal Loan Defaults are Resolved

Our records indicate that you are in default.

  • A Letter is needed from the lender/servicer or guarantee agency holding the defaulted loan(s) which provides clearance for each loan in default.
  • Students can view the status of their outstanding Federal Student Loan(s) by logging onto https://studentaid.gov/ and reviewing your Dashboard.


Exceeds Loan Limits

Our records indicate you have reached or exceeded the allowable limits for Federal Direct Subsidized and/or Unsubsidized Loans.

  • Your application for Federal Financial Aid has been suspended because you may be ineligible for Federal Financial Aid.
  • Contact the Arkansas State University – Mountain Home Office of Scholarships and Financial Aid for further assistance.


Letter Requesting Additional Information

Your application for Federal Financial Aid has been suspended because the Office of Scholarships and Financial Aid at Arkansas State University – Mountain Home has sent a letter requesting additional information.

  • Provide the additional information requested as soon as you receive the letter or contact the Office of Scholarships and Financial Aid at Arkansas State University – Mountain Home for further clarification.


Loans Discharged because of Disability

Persons granted a Total and Permanent Disability (TPD) Discharge are not eligible to receive a new loans in the future except under specific conditions with specific medical documentation.

  • The proper paperwork is available from your current loan servicer. Log onto https://studentaid.gov/ and review you Dashboard to determine the current holder(s) of your loans.


Marital Status Change

You corrected the date of your marital status to be after the date you submitted your application.

  • Your marital status and date should only be changed if they were reported incorrectly on the date the FAFSA was originally submitted, or if a Financial Aid Administrator has instructed you to update this information.
  • Review the FAFSA and make the necessary corrections or contact your Financial Aid Administrator for assistance.


Online - Direct Loan Entrance Counseling

Our records indicate that you have been offered a Direct Stafford Loan. One of the requirements which must be met to receive a this loan is Online Direct Loan Entrance Counseling available at https://studentaid.gov/entrance-counseling/.


Parent Signature of Federal Signature Page

The dependent student’s FAFSA was not signed by a parent. A parent signature is required on the Free Application for Federal Student Aid (FAFSA).

  • In order to submit or make corrections and sign the FAFSA online at https://studentaid.gov/ both the student and a parent must sign separately using their own Federal Student Aid (FSA) ID and password.  Apply for an FSA ID at https://studentaid.gov/fsa-id/create-account/launch.
  • Allow 3 to 5 business days for a new FAFSA to arrive at ASUMH.
  • Each update to a FAFSA can result in a request for additional information.


Pell Lifetime Eligibility

There is a limit to the total amount of Federal Pell Grants that a student may receive. Your records indicate that you are either close or have reached the maximum limit. Your Pell eligibility may be limited.


Unusual Enrollment History (UEH)

Based upon data provided by the National Student Loan Data System (NSLDS), additional information may be required to determine your eligibility for Federal Student Aid.

  • Contact the Office of Scholarships and Financial Aid for further assistance.

Important Disclosures about the Program and the Funeral Service Industry
I understand the program has a specific course rotation that I am required to follow each semester.
I understand that all Funeral Science Education curriculum course work must be completed with a grade of “C” or better.
I understand that I must maintain a 2.5 GPA and will only be allowed to retake courses from which I withdrew or failed in the first semester upon admission to the program.
I understand that I am responsible for any costs associated with the program including field trips and Training Camp.
I understand that it is my responsibility to know the licensing requirements for any state in which I intend to practice.
I understand that students who have previously been convicted of a crime may be restricted from certain clinical facilities and may be ineligible for some state licenses.
I understand that students who have a physical disability should contact the state in which he/she plans to seek licensure to determine if the disability will affect licensure.
I understand that a current driver’s license is required prior to enrolling in a clinical or practicum course.
